How the membership works:
At your first appointment you pay for all units injected at that appointment, only $3.48 per unit.

Then starting the following 1st of the month we will start your monthly payments of only
$58 per month for 12 months.

Dysport Information:
* Cost based on 4 individual appointments of up to 50 units of Dysport per quarter
* Membership protects you from price increases during your term
* Monthly auto payments will be drawn on the 1st of each month
* All units must be used within your membership term
* Client must sign a 12 month agreement
